The analogy with the meat packing place only holds if the woman and the man both forgo birth control. (I.e. they both know the meat is tainted, and that people could get sick as a result) If the woman lies about using birth control, this is more like her actively putting something in the meat to taint it. (Yes, the meat COULD have been tainted on its own, but she is actively encouraging this.) Ditto if a man lies about birth control ( lies about a vasectomy, perhaps) . However, even in the meat store analogy, even if the meat plant intentionally tainted the meat (was the one lieing about birth control), the meat store owner would be even more liable if she sold the meat to consumers, knowning this had happened. She knew the meat was bad, even if it wasn't her fault, and she still made the choice to sell it to the people, who are now blind.

Now, let's consider the case where she actively taints the meat (lies about birth control), and then sells the meat to people. Is there an way she is not 100% percent responsible?

If that's too strong, then let's say that lying about birth control is more like dropping the meat on a dirty floor out of carelessness. The analogy still holds true.
